The entries are made on facing pages, 1 made by the day staff, 1 by the night staff. The record the numbers of patients, their distribution (at school, occupational therapy, working on site or off-site, on other wards, sick, on leave, etc.), total numbers, names of patients receiving medication, and anything else of note, e.g. patients working late, on leave staying with their families, any particuar medical or behavioural problems, etc. The pages are signed by the nurses on duty.) |
